SYAG Registration Form - If filling out this form for baseball or softball, both page 1 (registration info) and page 2 (bat rule acknowledgement) must be completed and signed.  All other activities only need to complete page 1.

Accident / Incident Form - Part 1 to be completed by SYAG coach/director.  Part 2 to be completed by parent/guardian/participant, and mailed directly to the insurance company.

** Note ** You can enter information directly onto the claim form, and print it for mailing.  It cannot be submitted electronically, and information entered will not be saved when closing the form.  Right-click the link above to save the form to your computer.
